How is new medication invented?

I understand the stages of drug testing, from animal trials to supervised human trials to widespread testing etc. But how is the drug invented in the first place? I'm sure chemists have a better method than randomly synthesizing new molecules and injecting them in rats and observing the effects. So what is the process designing/inventing a new drug prior to testing?
